COVID-19 GUIDELINES - PLEASE REVIEW CAREFULLY

Please review the following COVID-19 requirements for the upcoming season:
- You will not bring your child to practices or games if they have any of the symptoms of COVID-19 (Fever, cough, shortness of breath, etc.)
- You understand that any child who appears sick to the coaches will not be permitted to participate, and will be required to leave the facility/field.
- You will not bring your child if they live with or have been in contact with any person who has tested positive for COVID-19 within 14 days of attendance or anyone currently exhibiting symptoms of COVID-19.
- You consent to have your child's temperature taken via a no touch temporal thermometer upon arrival. Anyone with a temperature of 100.4+ will not be admitted.
- You will provide your child with a drink that is marked with your child's name and is not be be shared.
- You consent to having hand sanitizer provided to your child before, during and at the conclusion of each session.
- You understand that if your child becomes sick during a practice or game that they will be removed from the group.
- You understand that there are risks of exposure to viruses to your child arising out of participation and hold NJ Millers harmless per their waiver.
- You have and/or will review all of the items set forth here with your participating child, and acknowledge, to the best of your ability, that your child understands these requirements and will fully comply.
- You understand that the procedures are subject to change, without notice, based on direction from Federal, State and Local authorities.
